Measuring Acrylic and Creating a Template |

1. Call the Part Department at 1-800-554-8268 - A template is
not always required, but dimensions are. |

2. Get a large piece of paper (12" x 36"). Wrapping paper is
excellent. |
| |
|
3. Place paper on hard surface. Stand acrylic on end and
carefully trace around with fine pen or pencil. Must be
|
|
exact drawing. |
| |
|
4. Measure width with flexible tape on the lamp side, must be
exact. |
| |
|
5. Measure length from end to end. |
| |
|
6. Write down Make, Model, Serial Number and Manufacturers
date of your bed. This information is usually on the |
|
metal plate on the back or under side of unit. |
| |
|
7. Any special features - i.e. facial unit, tape on ends, must
be noted and measured exactly. |
